Service:

Deals with the staff.  How polite were they? How quick or responsive were they to any of your needs?  Takes into account any service related issues.
10 - 9: Just about perfect in all aspects
8 - 7: Just misses the mark in one aspect, but solid overall. 
6 - 4: Average to below average in a couple of aspects, but nothing disastrous.
3 - 1: A complete failure in one or more aspects.
